PLAN DETAILS:
- Definition of Basic Cleaning: Standard cleaning that is completed in the absence of periodontal disease. Additional treatment fees may be added depending on periodontal health.
- Length of Contract: Agreement is for one year and paid in full at activation and is non refundable.
- Not Dental Insurance: This is a direct agreement for dental services by Crown Point Family Dentistry, will not be valid at any other dental offices and cannot be combines with dental insurance for additional discount.
- Exams, X-Rays, Cleanings, and Fluoride: Must be performed within the year of enrollment and cannot be carried over to the next year.
- Dental Services Only: This plan is for dental services only, not products.
- Payment: Payment is due at the time of services for any treatment not covered at 100% by plan, unless otherwise agreed upon by doctor and patient.
- Non-Transferable: Plan cannot be transferred to or from any other family member or friends.
- Rates are subject to change at any time.
